Student Health Insurance

The following is a memo about the College of Dentistry Health Requirements:

Re:        College of Dentistry Health Requirements

As part of the educational process in the College of Dentistry, you will interact daily with a variety of patients. Because of this patient contact, and for your own well being, you will be required to (1) purchase health insurance coverage and to have it in force during your entire tenure in the College of Dentistry; and (2) have a complete and accurate medical history and immunization record. (Please see enclosed forms.) Immunization records should be submitted to the College of Dentistry. They will then be forwarded to Student Health Services where, during the month of October, all first-year students will participate in a required health screening. A medical history form will be completed and immunizations reviewed at that time.

You must provide evidence of the following:

  1. Measles: Two doses of vaccine after your first birthday or presence of serum antibodies.
  2. Mumps and Rubella: Vaccination after your first birthday or presence of serum antibodies.
  3. Tetanus/Diphtheria: Immunization required within the last 10 years (Due to shortage of vaccine, is recommended, but not required.)
  4. Varicella (Chickenpox): Proof of immunity through immunization with two doses of vaccine, presence of serum antibodies, or physician statement of previous infection.
  5. Hepatitis B: Immunization or presence of serum antibodies. Immunization for Hepatitis B involves a series of three injections. The second dose is given one month after the first injection - the first two doses of Hep B vaccine should be AT LEAST a month apart to asure adequate response between doses -  the third dose is given 5 months later. Prior to enrollment, all students must have received the first two (2) injections in the vaccination series. The third injection must be completed by the end of winter quarter and documentation of the series completion must be provided to the College of Dentistry Health and Safety Office.  (Note more than one physician record is acceptable for Hep B immunizations.)

After completing the vaccination series, you will be required to provide proof of antibody to hepatitis B. The antibody test should be done 1 – 2 months after the 3rd dose of vaccine.

Tuberculosis: Intradermal PPD testing is required annually for all students who have not previously tested positive. This testing will be done on Friday, 27 September by Student Health staff.  (The cost of the test is $17; however, Student Health Insurance covers 1/2 the cost - your final cost ~$8.50.) The test will be read on the following Monday, 27 September. Option - you may choose to have the PPD test done by your family physician.  If you have previously tested positive for tuberculosis as a result of infection or vaccination, you must provide physician documentation of the date and results of testing, verification of chest x-ray results, and a record of any treatment that you received.

Health Insurance: Print, complete and sign the Health Insurance Agreement form.

Your Immunization and Lab Studies (PDF Format | Rich Text Format) and Health Insurance Agreement forms must be completed and returned to The College of Dentistry Admissions Office by Friday, 30 August 2003. In keeping with the policies of the College of Dentistry and requirements by affiliated hospitals, students who do not provide this information will forfeit their position in the class.
